2.0.0.3. The Board may collect information under section 2.0.0.2, in particular, from(1) the provider concerned, in the cases determined by regulation of the Government;
(2) the professional order concerned, in the case of a provider whose profession is governed by the Professional Code (chapter C-26); (3) a person designated by a competent authority within the organization for which the health and social service provider exercises functions or practises a profession; and
(4) an access profile manager referred to in section 520.3.8 of the Act respecting health services and social services (chapter S-4.2), in the case of the persons employed by or under the direction of that manager and in respect of whom the latter may make a request under subparagraph 2 of the first paragraph of section 520.3.3 of that Act enabling those persons to obtain and use a certificate. The persons and body referred to in subparagraphs 1 to 4 of the first paragraph must communicate the information referred to in section 2.0.0.2 to the Board, and must thereafter inform the Board without delay of any change in that information.
