1. Application for the issue or renewal of a permit to operate an institution shall be made to the Minister of Health and Social Services by filling the form prescribed in Schedule I.

Application for the issue of a permit must be accompanied by a statement of the objects pursued and of the means to attain them. In the case of a legal person, a resolution of the board of directors authorizing the application must be attached to the form and, in the case of a partnership or a person operating an enterprise under a name which does not include the person’s surname and given name, a certified copy of the declaration required under the Act respecting the legal publicity of enterprises (chapter P-44.1) dated less than a month before the date of the application must also be attached.
