108.13. The appeal must be brought to the commissioner within thirty days of the decision reconsidered by the board or of the expiry of the delay provided for by section 108.12, by way of a notice setting out

(a)  the name and domicile of the applicant;

(b)  the date and nature of the decision of the board;

(c)  the relevant facts; and

(d)  the conclusions sought.
