58.4. Every application to the competent board of revisors must be made under oath.

The board may require the person making an application to submit to it any proof necessary for the making of a decision. However, in the case of an application to have the name of a person domiciled in the territory of the English-language school service centre entered, the board shall require from the person making the application the former address of the domicile of the person in respect of whom the application is being made as well as two documents, one showing the name and date of birth of the person in respect of whom the application is being made and the other showing the name of that person and the address of the person’s domicile.
