3. The petition shall mention
(a)  the names, head offices and modes of constitution of the petitioning parochial bodies;
(b)  the name and head office of the company to be constituted;
(c)  the powers to be assigned to the company in addition to those hereinafter mentioned in section 23, as well as the rules governing their exercise;
(d)  the name and the address of the person designated as provisional chairman of the company.

(a)  the names, corporate seats and modes of constitution of the petitioning parochial bodies;
(b)  the name and corporate seat of the corporation to be constituted;
(c)  the powers to be assigned to the corporation in addition to those hereinafter mentioned in section 23, as well as the rules governing their exercise;
(d)  the name in full and the address of the person designated as provisional chairman of the corporation.
