6. A government department or organization referred to in section 5 must, in particular, indicate in its service statement(1) its name and the address of its head office;
(2) a description of its mission;
(3) a description of the services it provides for persons who are victims;
(4) a list of its commitment towards persons who are victims;
(5) a description of its complaint mechanism, presenting(a) the person responsible for receiving complaints;
(b) the procedure for filing a complaint;
(c) the right of a person who is a victim to be informed of the outcome of the complaint;
(d) the time needed to process a complaint.
