16. An application for the review of a decision of the Minister must be filed in writing and contain, in particular, the following information and documents:(1) the name and contact information of the person who is a victim and, where applicable, of their representative;
(2) the date of the contested decision;
(3) the object of the contested decision;
(4) the main grounds in support of the contestation;
(5) the documents or other elements that will be presented;
(6) if the application is filed after the time prescribed for doing so, the reasons for the delay.
