9. A return of birth within the meaning of section 45 of the Public Health Act (chapter S-2.2) must be completed at each confinement.
For live births, Form SP-1 appearing as Schedule 1 must be completed.
For stillbirths, miscarriages or therapeutic abortions, Form SP-4 appearing as Schedule 4 must be completed if the foetus weighs 500 g or over.
Every completed form must be mailed, in conformity with section 48 of the Act to the Institut de la statistique du Québec, no later than 8 days after confinement.
