3. Any candidate for the practice of dentistry wishing to register for the examination of the Order shall, not later than 60 days before the date set for an examination:

(1)  give notice of his intention to sit for the examination by filling out a registration form provided for that purpose and sending it to the secretary of the Order together with any documents required;

(2)  have paid the costs related thereto determined by resolution of the board of directors pursuant to paragraph 8 of section 86.0.1 of the Code.

In the case of a supplemental examination referred to in section 14, the candidate shall in addition fill out and send to the secretary of the Order the prescribed attestation stating that he has completed, according to the prescribed standards, any additional period of training required.
