21. To qualify for participation, a foundation and a trust must file an application with Épargne Placements Québec on the prescribed form, and submit all required documents.

The form must be completed by the natural person or persons acting as authorized administrators of the foundation or trust and be filed with documents attesting to capacity as an authorized administrator.

If 2 or more co-administrators are authorized to act, they may designate one or more of their number to complete the participation form and act in the participant’s name. The designation is made on the prescribed form and must specify, if applicable, which of the administrators act jointly.

If the administrator is a legal person, sections 17 and 18 apply, with the necessary modifications.
