8.1. The costs exigible for the replacement of a health insurance card before its expiry date shall amount to $25 for a replacement in person or by mail and to $15 for a replacement online.

The costs of $15 exigible for a replacement online come into force on 1st February 2015.
8.1. The costs exigible for the replacement of a health insurance card that has been lost, damaged or stolen shall be $23.
