DIVISION
I
WRITTEN PRESCRIPTIONS
1
.
A dentist shall not furnish prescription slips for medication, devices or treatments unless they clearly indicate:
(
1
)
his name, address and telephone number;
(
2
)
the date of the prescription;
(
3
)
the patient’s identity including his address;
(
4
)
his signature;
(
5
)
his licence number;
(
6
)
in the case of a treatment, the nature of the treatment;
(
7
)
in the case of a medication, the name of the medication, the pharmaceutical form, the concentration, the quantity prescribed or the length of treatment, the dosage and the number of authorized renewals;
(
8
)
in the case of a device, the kind and description of the device with, in the case of a removable prosthesis, a diagram of the device;
(
9
)
for a renewable prescription, its period of validity.
Subparagraphs 5 and 8 do not apply to prescriptions written at a hospital centre for a user to whom the Act respecting health services and social services (chapter S-4.2) applies or for a beneficiary within the meaning of the Act respecting health services and social services for Cree Native persons (chapter S-5).
