Regulation respecting the keeping and publication of the register of civil status
Civil Code of Québec
(Civil Code, art. 151).
DIVISION I
ADDITIONAL PARTICULARS TO APPEAR ON ATTESTATIONS OF BIRTH AND DEATH
1. An attestation of birth shall state, in addition to the information required by articles 110 and 111 of the Civil Code, the following additional particulars:
(1)  the code number of the institution where the birth occurred, where applicable;
(2)  the place of birth of the mother; and
(3)  the professional permit number of the physician who delivered the baby, where applicable.

2. An attestation of death shall state, in addition to the information required by articles 110, 124 and 128 of the Civil Code, the following additional particulars:
(1)  the code number of the institution where the death occurred, where applicable; and
(2)  the professional permit number of the physician who certified the death, where applicable.

DIVISION II
ADDITIONAL PARTICULARS TO APPEAR ON DECLARATIONS OF BIRTH, MARRIAGE AND DEATH
3. A declaration of birth shall state, in addition to the information required by articles 110, 115 and 116 of the Civil Code, the following additional particulars:
(1)  the dates of birth of the child’s father and mother; and
(2)  for the purposes of the declaration of filiation of the child, whether the child’s father and mother are married to each other and the date of their marriage, where applicable.

4. A declaration of marriage shall state, in addition to the information required by articles 110, 119 and 120 of the Civil Code, the following additional particulars:
(1)  the marital status of each of the spouses to be; if either is divorced or widowed, the date of the last divorce or the date of the former spouse’s death;
(2)  the place where the birth of each of the spouses was registered; and
(3)  the code number assigned to the officiant by the registrar of civil status.

5. A declaration of death shall state, in addition to the information required by articles 110 and 126 of the Civil Code, the following additional particulars:
(1)  the place where the birth of the deceased was registered; and
(2)  the marital status of the deceased.

DIVISION II.1
CERTIFICATES OF BIRTH, MARRIAGE, CIVIL UNION OR DEATH
O.C. 769-2023, s. 1.
5.1. The certificate of birth of a person shall state
(1)  the name of the person;
(2)  the designation of sex;
(3)  the place, date and time of birth of the person;
(4)  the names of the person’s father and mother or parents, as well as their parental designation;
(5)  an indication that the person has died, where applicable; and
(6)  the registration number of the act of birth.

5.2. The certificate of marriage or civil union of the spouses shall state
(1)  the name of the spouses;
(2)  the places and dates of birth of the spouses;
(3)  the place and date of the spouses’ marriage or civil union;
(4)  the cause of the dissolution of the marriage or civil union, where applicable; and
(5)  the registration number of the act of marriage or civil union.

5.3. The certificate of death of the deceased shall state
(1)  the name of the deceased;
(2)  the designation of sex;
(3)  the place, date and time of death of the deceased;
(4)  the place and date of birth of the deceased; and
(5)  the registration number of the act of death.
