P-9.0001 - Act respecting the sharing of certain health information

Disposition versions
79. The register of users contains the following information on every person registered:
(1) the person’s unique user identification number,
(2) name,
(3) date of birth,
(4) sex,
(5) address,
(6) health insurance number, if applicable,
(7) parents’ names,
(8) the name of the person’s legal representative and the dates on which representation by the latter begins and ends, if applicable,
(9) the person’s date of death, if applicable, and
(10) if the person is not registered with the Régie de l’assurance maladie du Québec, the number and title of an official document issued by a state authority establishing his or her identity.
79. The register of users contains the following information on every person registered:
(1) the person’s unique user identification number,
(2) name,

(5) address,
