101. (1)  Every application made under section 97 or section 100 must indicate the surname, Christian names, profession or occupation, age and address of the person who makes it and of the person in respect of whom it is made.
(2)  The oaths provided for by sections 97 and 100 must be administered by a revisor, the returning officer, the election clerk, any assistant election clerk, any of the assistants appointed under subsection 4 of section 83 or any other person specially appointed for such purpose by the director general of elections.
(3)  Every person in charge of receiving such applications must receive every application regularly made by an elector of the division concerned and give to all persons who file them a copy containing the following details:
(a)  the surname, given names, profession or occupation, age and address of the person making the application;
(b)  the surname, given names, occupation, age and address of the person in respect of whom it is made;
(c)  the surname, given names, occupation, age and address of any witness who attests such application, when such attestation is required;
(d)  the date and hour of the filing of the application.
(4)  The copy provided for in subsection 3 makes proof of the receipt of such applications.
