119. A declaration of marriage states the name and domicile of each spouse, their places and dates of birth, the place and date of their marriage, and the name of the father and mother of each of them and of the witnesses.
The declaration also states the name, domicile and quality of the officiant and indicates, where applicable, the religious society to which he belongs.
