98. The memorandum of constitution shall give all the information prescribed by regulation of the Government and specify the name of the contemplated association, the place of its head office, the surnames, names, occupations and residences of the signatories and of the person provisionally designated as secretary for the filing of copies of the memorandum with the Inspector General and the calling of the organizing meeting provided for in section 104, the mode and procedure of calling such meeting, and the fields of activity in which it is to provide coverage.
The memorandum shall be accompanied with the documents prescribed by regulation of the Government.
In addition, the memorandum shall be accompanied with specifications regarding the tariffs to be applied and the indemnities to be paid, and such specifications shall, moreover, be certified to conform to actuarial principles in a certificate bearing the signature of an actuary.
